Cleaning determines the success of the entire process — protein residue on an instrument makes sterilization impossible regardless of the method used. Decontamination therefore begins with a washer-disinfector providing thermal disinfection and a documented A0 value, supplemented for complex geometries by an ultrasonic washer that removes soil from holes and channels a water jet cannot reach. Flexible endoscopes require a separate washer compliant with endoscope reprocessing standards.

What does the A0 value mean and what is required?
A0 expresses thermal disinfection efficacy as exposure time normalised to 80°C. A0 ≥ 600 is assumed for surgical instruments, and A0 ≥ 3000 for devices contacting broken skin or mucous membranes.
Does an ultrasonic washer replace a washer-disinfector?
No. Ultrasound removes soil mechanically from hard-to-reach places but provides no thermal disinfection. It is a stage preceding the main wash, not a substitute for it.
Can the washer have double doors?
Yes. The pass-through version with doors on both sides separates the dirty and clean zones, which is standard in central sterile supply departments and eliminates the risk of re-contaminating the load.
